Our Clinical Care Coordinators

Our Clinical Care Coordinators are registered nurses who have one goal: to make sure you have the support you need. Our team includes:

Margarita Asiain, RN received her undergraduate degree and Registered Nursing license in California and received a Bachelor of Science in Nursing at Pace University after moving to NYC. She is Case Management Certified and is currently pursuing a Master of Science in Nursing and a Master in Public Health at City University of New York at Hunter College. Margarita's clinical experience includes hospital/bedside nursing in a cardiac step-down unit and in home health care for a Managed Long Term Care Program providing services to Medicaid recipients in NYC. In home health care, she worked as a care coordinator in the field and as a staff developer. Her interests are in population health, preventative health care, community-partnership development, health care policy, and reduction in health care disparities.

Shakespear Boka, RN received a Bachelor of Science in Nursing from York College City University of New York.  He is a Certified Case Manager and is currently pursuing a Master of Science degree. Shakespear is also an American Heart Association Certified BLS instructor, Medication Administration Trainer and Mandated Reporter Trainer for New York State.  Shakespear’s clinical experience includes direct patient care and supervisory roles in acute care (medical-surgical), long term care, home health care, and case management.  Current professional interests include nursing informatics, health-care advocacy, wellness coaching, care management, and community-partnership development. 

Gael Donchance, RN received a Bachelor of Arts in Psychology from City College of New York before pursuing a Bachelor of Science in Nursing from Long Island University. She has nursing experience caring for post-operative orthopedic and neurosurgery rehabilitation patients at the Hospital for Joint Diseases. There she was a member of the Evidence Based Practice Council and actively worked on quality improvement projects. Prior to nursing  she worked as a Research Assistant on National Institutes of Health funded studies.

Rosaria Ferranti, RN, BSN, ONC received a Bachelor of Science in Nursing from the LIU School of Nursing, and is a certified Orthopedic Nurse. While working as a nurse on the orthopedic unit at the Hospital for Joint Diseases, she was on the Unit Practice Council and a member of the Coordinating Council. Rosaria is currently pursuing her Nurse Practitioner/Clinical Nurse Specialist degree at the College of Staten Island.

Patricia Finnerty, RN received a Bachelor of Science in Management from Purdue University and graduated from Pace University with a Bachelor of Science in Nursing. Before pursuing nursing, Patricia had a career in marketing and conference services management. Her nursing experience has been in caring for heart patients at Tisch Hospital.  She is RN board certified in Cardiac-Vascular Nursing and fluent in Spanish.           

Shobha Ganesh, RN earned a Bachelor of Arts in Psychology from Long Island University before pursuing a Bachelor of Science in Nursing at Wagner College. She is currently pursuing a Master’s Degree in Adult/Geriatric Nurse Practitioner at Long Island University. Shobha’s nursing experience includes obstetrics and gynecology, prenatal counseling, pediatrics, medical surgical and home health care management. She has also spent many years  providing care for underserved communities within the five NYC boroughs. Shobha continues to advocate for improvement in quality of care.                       

Millicent Hamilton, RN, ANP has been practicing nursing for the past 17 years. She received her AAS degree in nursing from Eugenio Maria de Hostos community college and her BSN in nursing from the College of New Rochelle. She went on to pursue her graduate studies at SUNY Stony Brook University majoring in adult health and a concentration as an Adult Nurse Practitioner. She is board certified and practiced as a nurse practitioner in internal medicine. She had worked in a number of clinical areas and had held managerial positions. Special projects include the creation of a state approved psychotropic injection program for the St. Vincent Health System. She currently conducts various health education classes on primary prevention and is looking forward to pursuing her DNP in the near future.

Octavia Harrison, BSN began her Nursing career as a Special Needs Nurse in the Community/Public Health Sector.  Octavia received her Bachelors of Science Degree in Nursing with a Minor in Biology from the College of Mount Saint Vincent Riverdale, NY.  Octavia has a passion for meeting the needs of others. Her main goal is to always try and return the patient to their previous state of stability. Patient education is of high importance to Octavia.   Thus, she is currently completing a Master’s of Science Degree in Nursing with a concentration to become an Adult Health/Gerentological Nurse Practitioner at Hunter College. Octavia is also preparing to complete a Post Masters Certificate in Nursing Education. Other clinical areas of Octavia’s experience include medical/surgical, hospice/palliative care and home health care.             

Erin McKay, RN has been a Registered Nurse for thirty years.  She began as a medical-surgical nurse working in the areas of oncology, interventional cardiology, and cardiovascular surgery.   Her career has included positions in commercial health insurance medical management and as a Director of Nursing at a skilled nursing facility.  Erin joined NYULMC almost sixteen years ago.  Before becoming a Clinical Care Coordinator, she held positions in utilization review, care management and finance as a clinical charge auditor.                     

Michelle Palazio, RN holds a Bachelor of Arts in Psychology from Binghamton University and a Bachelor of Science in Nursing and a Master of Science in Education in Psychological Services from the University of Pennsylvania. Before pursuing nursing, Michelle’s career included positions in human resources, behavioral research for a large teaching hospital and psychological counseling as a therapist. Her past clinical nursing experience includes renal care, pediatrics and ambulatory care. She is a certified pediatric nurse and fluent in Spanish.  She is currently the Manager of Clinical Care Coordination for the CARE program.            

Lauren Raichle, RN is a graduate of Indiana University where she received a Bachelor of Science in Nursing. She has nursing experience caring for post-operative orthopedic patients at the Hospital for Joint Diseases. She has co-chaired and participated in patient education committees and safety initiatives. Educating patients, families and nursing staff has been a constant thread throughout her career. Lauren also has volunteer experience in a skilled nursing facility and an emergency department.                  

Susan Salz, RN has been a registered nurse since 1988. She received a BSN from Columbia University. She is a native New Yorker, having grown up in Sunnyside, Queens and having lived in Manhattan most of her adult life. Susan’s professional career has included oncology, pediatrics, rehabilitation, hospice and palliative care, and case management. She has certifications in case management and in hospice and palliative care nursing. Susan has a particular interest in global health; she spent this past August in East Africa, volunteering at a hospital in Uganda. She plans to pursue graduate education in public health. Susan is fluent in Hungarian.

Judith Scott, RN received a Bachelor of Business Administration and had a business career in marketing and customer service before completing a nursing degree at the Borough of Manhattan Community College.  She currently is pursuing a Master of Science in Nursing. Judith’s nursing experience includes sub-acute rehabilitation, home care and home infusion. She held a supervisor position at a skilled nursing facility and was a home infusion liaison for staff at multiple hospitals including Tisch Hospital.      

Christine Truiano, RN received a Bachelor of Science in Management & International Business from Manhattan College, and worked for Viacom Inc, before perusing a Bachelor of Science in Nursing from Pace University.  She went on to obtain a Master of Business Administration from Northeastern University.  Her career has included positions in Pediatrics, Orthopedics, Surgical Head & Neck Oncology and Apheresis.  Before becoming the Manager of Care Coordination for the Network Integration Department, Christine was Senior Clinical Care Coordinator at Hospital for Joint Diseases.

Betty Viaud, RN, MHSA received a Bachelor’s of Health Services Administration and a Masters of Health Services Administration. Betty's nursing experience includes pediatrics, ambulatory care, home health care management and Managed Care case management.  Before pursuing nursing, Betty worked as a quality assurance analyst working on multiple information technology platforms. Current professional interests include pursuing a Master of Science focusing on family health and care management.
