Support Through Care Coordination

NYUPN Clinically Integrated Network is positioned to deliver high-quality, patient-centered, accessible care to its patients, and become a national leader in healthcare delivery system innovation. To achieve that goal and help you deliver the best possible care and support to your patients, we have developed a care coordination program.

Care coordination is a system-wide approach to improve your patient’s health by partnering with all members of their care team, and helping patients navigate the complex health care system. Care coordination services are offered by our Clinical Care Coordinators, registered nurses who are part of a physician-led team.

Our Clinical Care Coordinators can assist your patients in obtaining primary and secondary care, providing ongoing patient education, identifying potential medication complications, and managing complex or chronic medical conditions. 

As a NYUPN physician, many of your patients who have a medical health insurance benefit plan with Cigna, UnitedHealthcare, Oxford, Aetna and Aetna Medicare Advantage can work directly with a Clinical Care Coordinator for personalized one-on-one support to help manage their health and establish health goals in line with the care you are providing them. Additionally, NYUPN is participating in Medicare’s Bundled Payment for Care Improvement initiative; patients undergoing specific surgeries, such as heart valve surgery, spinal fusions, or hip, knee, or ankle replacement surgery, are also able to utilize our care coordination services.
